All Faiths offers integrated, trauma-informed behavioral health care for adults, including specialized services for veterans. Our coordinated approach addresses both mental and physical health — supporting timely access to care, consistency across services, and long-term wellness.

Outpatient Therapy and Substance Use Treatment

WHAT TO EXPECT WHEN YOU REACH OUT
Accessing care should be straightforward, supportive, and centered on your needs. Whether you’re seeking services for yourself or referring someone in your care, our team is here to guide you through every step of the process with professionalism and compassion.
Step 1: Initial Contact and Scheduling
Once we receive a referral or inquiry, a member of our Client Access and Engagement team will follow up to gather essential information, answer questions, and assist in scheduling the next steps. Our goal is to ensure a smooth and welcoming entry into care.
Step 2: Primary Care Screening
As part of our integrated model, clients receive a Primary Care Screening conducted by our Nurse Practitioner. This screening allows us to identify any physical health concerns that may impact behavioral health and ensures we are addressing each client’s needs comprehensively.
Step 3: Clinical Intake Assessment
Clients are also scheduled with a Clinical Assessment Therapist for a comprehensive, trauma-informed evaluation. This session helps us understand each individual’s history, strengths, and goals, and serves as the foundation for a personalized plan of care.
Step 4: Connection to Services
Following the intake assessment, clients are connected to appropriate services based on their care plan. These may include outpatient therapy, intensive outpatient programming (IOP), peer support, or substance use recovery services. When applicable, we coordinate with outside providers and support systems to promote seamless, ongoing care.
Step 5: Ongoing Communication and Support
Our team remains engaged throughout the course of care, offering continued support and adjusting services as needed. With consent, we maintain communication with referral partners to ensure coordination and continuity of care.
